Choose the healthy activities you want to complete. Each completed activity below is worth reward points (example, 500 points = $50). It can take up to three days to receive your link to redeem your points. Earn up to 1,500 points for a maximum of $150 per calendar year. Adult children (ages 18 and older) covered under a family member's policy can earn up to 1,000 points for a maximum of $100 per calendar year. Rewards come in the form of gift cards to your choice of national retailers and restaurants. All points must be redeemed by Dec. 31, 2020. SSM Health employees are not eligible.
The health assessment asks lifestyle-related questions to evaluate your overall well-being. By completing your online health assessment, you’ll unlock additional reward points for additional healthy activities.
Log in to your WellFirst Health member portal. If you are a first-time user and have your member number from your ID card handy.
After you are completely logged in to the portal and you have selected your WellFirst Health plan, click the 'LIVING HEALTHY' link. A new WebMD One tab will open.
Before you can start the Health Assessment, WebMD One will ask you several one-time only questions.
First-time WebMD Living Healthy users will need to register.
On the WebMD Living Healthy homepage, click the 'Health Assessment' link in the menu in the upper right corner.
Enter your most recent health screening measurements into the health assessment. You can find these measurements from an onsite screening if offered by your employer or from a clinic visit by logging into your MyChart account.
After you’ve answered all the questions and entered your health screening measurements, click “Finalize” to complete your health assessment.
After finishing your health assessment, log any additional healthy activities you have completed.
Log in to your Living Healthy portal and click the “Rewards” tab.
Click “Log it” next to the other healthy activities you have completed. Please provide any additional information requested.
Complete one My Health Assistant goal per calendar quarter to earn the maximum points.
Complete a health topic self-assessment once a month to earn the maximum points.
After logging your activities, you will receive an important email from WellFirst Health that you will use to access your Living Healthy reward. It can take up to three days to receive your link to redeem your points. Your reward comes in the form of gift card(s) to your choice of many national retailers, restaurants and other popular merchants.
Click the link in the email, which takes you to a website that displays available gift card merchants. Select the gift cards you wish to receive. You can even split your reward in different amounts to different merchants.
Once you have selected your gift card(s), click “Checkout” and enter your mailing address.
Click “Place Order” to finish and you will receive your gift card(s) by U.S. mail. Please allow up to 14 business days.
Be sure to complete all activities and redeem your points by the end of the reward year–Dec. 31, 2020.
If your coverage ends Dec. 31, 2020, be sure you submit your assessment, complete your activities and redeem your points before that date.

Rewards for covered adult children Covered adult children (ages 18 and older) can earn up to $100 per year for completing healthy activities, however, they are not required to complete the Health Assessment. Rewards for this activity are excluded from the rewards program (formerly a $50 value) due to changes made by the Equal Employment Opportunity Commission (EEOC) Final Rule on Employer Wellness Programs and the Genetic Information Nondiscrimination Act (GINA). Covered adult children may continue and are encouraged to complete the health assessment within their Living Healthy portal for their own benefit, however, regulations prevent WellFirst Health from rewarding this activity as of Jan. 1, 2017.
Check with your plan administrator for reward offerings specific to your plan WellFirst Health reserves the right to validate each healthy activity you submit for reward points at any time. Only WellFirst Health members age 18 and older are eligible for Living Healthy rewards. Your employer may be required to report health plan-issued incentives as taxable income. WellFirst Health is required to report incentive payment information to your employer. Your health information is protected by federal law and will not be shared with your employer.
Redeeming your points All points must be redeemed by Dec. 31, 2020. SSM Health employees are not eligible. Be sure you submit your assessment and complete your activities before that date. It can take up to three days to receive your link to redeem your points. If you wait too long to complete your activities, you might not receive your points before the current reward year ends.
Health First Rewards Plan (HMO) H1099-014 is a 2018 Medicare Advantage or Medicare Part-C plan by Health First Health Plans, Inc. available to residents in Florida. This plan includes additional Medicare prescription drug (Part-D) coverage. The Health First Rewards Plan (HMO) has a monthly premium of $0.00 and has a in-network Maximum Out-of-Pocket limit of $6,650 (MOOP). This means that if you get sick or need a high cost procedure your co-pays are capped once you pay out of pocket $6,650 this can be a very nice safety net.
Health First Rewards Plan (HMO) is a Local HMO. With a health maintenance organization (HMO) you will be required to receive most of your health care from an in-network provider. Health maintenance organizations require that you select a primary care physician (PCP). Your PCP will serve as your personal doctor to provide all of your basic healthcare services. If you need special care for a physician specialist, your primary care physician will make the arrangements and tell you where you can go in the network. You will need your PCPs okay, called a referral. Without getting a referral or services received from out-of-network providers are not typically covered by the plan.
Health First Health Plans, Inc. works with Medicare to give you significant coverage beyond Part A and Part B benefits. If you decide to sign up for Health First Rewards Plan (HMO) you still retain Original Medicare. But you will get additional Part A (Hospital Insurance) and Part B (Medical Insurance) coverage from Health First Health Plans, Inc. and not Original Medicare. With Medicare Advantage your always covered for urgently needed and emergency care and you receive all of the benefits of Original Medicare from Health First Health Plans, Inc. except hospice care. Original Medicare still provides you with hospice care even if you sign up for a Medicare Advantage Plan.
